  • Interest income statements (Form 1099-INT & 1099-OID)
  • Retirement plan distribution (Form 1099-R)
  • Proceeds from broker transactions (Form 1099-B)
  • Early withdrawal penalties on savings and CDs

Homeowner or Renter Information

  • Your home address for the current tax year
  • Real estate taxes paid
  • Mortgage interest – Form 1098
  • Second mortgage interest
